It's a cosmetic mechanic to see how much of your population likes you (because you are the leader despite what type of government you chose) The more actions you do to gain the favor of your people (winning wars, improving the economy, etc.), the higher it goes. The more actions you do/are done to you (having war declared on you, losing wars, every new turn you don't have food, higher disease rates, etc.)
